A new helpful system we have been using is called 'MEDLINK'. This is a range of on-line questionnaires regarding your health.

Individual, specific questionnaires are available for health updates and Chronic Health Conditions such as: Asthma, Diabetes, CHD, COPD, Thyroid Disease etc. 

They also include links to very useful and informative videos relating to your condition which will assist you in aiding your health.

When your annual review is due and you are elligible for a MEDLINK review you will receive a text message or email informing you of this. A link to the Medlink questionnaire, appropriate to you, will then follow in another text message / email. 

Please complete this as promptly as possible. When the health centre receives the completed questionnaire it will be reviewed by the appropriate clinician. 

You will then receive a text or email to advise on further action, which can be:

- To arrange an appointment with the health care assistant for further tests

- To arrange a telephone consultation or appointment with the GP

- Or NO further action required
