THE PRACTICE IS OPERATIONAL AND WE ARE OPERATING A TRIAGE SYSTEM, PLEASE TELEPHONE FIRST.  SHOULD YOU REQUIRE A PRESCRIPTION WE CAN ARRANGE FOR THIS TO GO TO A CHEMIST, PLEASE ALLOW MORE THAN 48HRS DUE TO THE HIGHT DEMAND AT THIS TIME. 


 

WUHAN NOVEL CORONAVIRUS

If you have returned from Wuhan, Hubei province or mainland China or travelled from Italy in the last 14 days or you have been in contact with a confirmed case of Novel Coronavirus please do not come into the surgery please telephone first on  221976 or outwith surgery hours call NHS 24 on 111.  General information can be found at  https://www.nhsinform.scot/ or you can call the Novel Coronavirus Helpline number on 0800 028 2816.

Should I see the Doctor or Nurse Practitioner?  Nurse Practitioners are highly trained specialist nurses who have completed further education and training so that they can provide advanced nursing care and prescribe medication.  They can provide treatment and advice for many of the problems that traditionally you saw your Dr about in the past.  If you are unsure if they can deal with your problem please ask at Reception.

Pre-registration with the practice

PreRegistrationIf you wish to pre-register click on the link below to open the form. When you have completed all of the details, click on the "Send" button to mail your form to us. When you visit the surgery for the first time to complete the registration process you will require two forms of identification preferably your passport, driving licence and or a utility bill. You will be asked to sign the form to confirm that the details are correct.

Please note you will not be registered with the practice until the forms are completed, signed and proof of identity has been verified.

When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records. There is an online version of this file too, which you may fill out and send to us. When you come to the surgery you will be asked to sign this form to confirm that the details are correct.

Note that by sending the form you will be transmitting information about your self across the Internet and although every effort is made to keep this information secure, no guarantee can be offered in this respect.


Alternatively you may print off a registration form, fill it out and bring it in with you on your first visit to the practice.
